SEC. 37-23-155. Announcement of program; manner and form of reporting.
(1) In accordance with the provisions of section 37-23-153, the director shall provide reporting forms and prepaid return envelopes to each physician or optometrist licensed to practice, and to each institution having contact with hearing-impaired or visually-impaired children in the State of Mississippi. It is the purpose of this section to facilitate the reporting of persons who are suffering from impaired hearing or vision. The reporting form required by this section shall be as simple as possible and shall be designed to be completed by nonprofessional persons.
(2) The director shall produce announcements of the program which set forth the purpose of the program and which invite registration from the public at large.
(3) No physician, optometrist, or participating institution shall be required to report an individual more than one (1) time nor shall they be required to report any individual they have reason to believe has been previously reported. The reporting form shall contain a provision for the release of such information, including a release to the state department of education, the Mississippi School for the Deaf, and the Mississippi School for the Blind, and a waiver of the physician-patient privilege and shall not under any circumstances be required without the written consent of a parent, guardian or a person in loco parentis to the child who is the subject of the reporting form.
