SEC. 93-11-59. Form for Uniform Reciprocal Enforcement of Support Act action request; uniform support petition; paternity affidavit.

The form of the URESA action request, uniform support petition and the paternity affidavit, if applicable, as required to initiate an action for support under Sections 93-11-1 through 93-11-63, shall be in accordance with the standardized URESA forms or replicas thereof as required by the United States Department of Health and Human Services, Family Support Administration, Office of Child Support Enforcement or its successor. Upon request, the State Department of Public Welfare shall make these forms available at a minimal cost.
